AI and "Human in the loop" - what does that actually mean in practice?
Every AI agent pitch I see includes this phrase somewhere. Human in the loop. Human oversight. Human supervision.
But when I look at how it actually works inside most companies, it breaks down into one of three things:
A person reviews the output after the action already happened. A person could intervene but the system makes it slow and inconvenient. One person monitors a dashboard tracking 40 agents running tasks they do not fully understand.
None of that is oversight. It is a paper trail.
The problem is not that people are lazy or careless. It is that most agent systems are built for speed first.
Oversight gets added later to satisfy a compliance requirement or a nervous investor. The human is on the org chart but absent from the actual decision.
I have been thinking about this while building my own platform. We have one agent running most of our operations. I am the only person with authority to shut it down. That was a design choice, not an afterthought.
Three questions I want to put to people who actually build or deploy these systems:
What does real oversight require? Veto rights? Visibility into the reasoning? Full audit trails? Something else?
Is there a point where adding a human to the process creates false confidence rather than actual control?
Has anyone seen an AI agent design where oversight is genuinely built in from the start, not added on top?
