It is the thing I struggle with the most. I usually get up grab a cup of coffee and get back to work but it doesn't always do the trick for me. So what are your tactics?

If I get into a rut, I pre-plan the night before the ONE thing that I am going to accomplish. Just one. Then after I do that one thing, 9 times out of 10 I'm looking for something else to go ahead and do.
Projects can get overwhelming, and breaking them down into easy to manage items that can help you ramp up. You don't have to break the entire project down, but when you get to a section, having simple to solve issues helps get you in the zone to do more. Just my 2 cents!

what triggers you to stay focused
what triggers you to go the extra mile
what triggers you to...
if you have the end in mind, the feeling you get. that should be your focus.
example, eating makes all the blood go to your stomach. if you eat a lunch that needs much energy to transform into energy you will feel sleepy, less focussed. so what you can do is eat a heavy breakfast before commute and sleep on the train. Or steam vegetables the night before, take them to work and eat them for lunch. you will NEVER have this afternoon dip.
another example. sometimes I have a job I really don't wan't to do. I think about how free I feel when it's over. so then I want that feeling more than ever and I get my shit together.

Just step away for few minutes, and come back with focus
Getting a cup of water (or coffee, if the problem is big :D ) Then having a look at my to do list with the deadlines usually puts enough pressure to snap back into reality. :)
Lol i just go out with a friend to chat and forget about work for that moment.

For me, the first half of the day is usually very productive, and I notice that after lunch my productivity starts dropping and I become more easily distracted. What I do to get myself back on track is to step outside and breath some fresh air, relax my eyes from all devices I use and clear my thoughts. When I get back at my desk, before I open my laptop, I take my notebook out and write down the tasks I need to finish for the rest of the day. I try to be very precise and detailed when writing these tasks. This really helps me to keep my attention on one place at a time and I usually am able to go about my day without a problem.
