Accuracy problem usually comes from 1) lack of data - hallucination; 2) too much data - wrong attention; We improve accuracy by retrieving and structuring information into memory graph upon happening, just like introducing a real time index to a book while writing the book. In that way, even when there's too much data, we can find the right information correctly. Same works for the scenario when there's implicit context (which in traditional cases would be categorized as lack of data).
